Please note that all new patients are required to complete the new patient registration process prior to their office visit. We have made this process completely electronic to aid the reduction of paper waste and make our office more efficient. You will receive an email or text message from us 3 days prior to your appointment to ensure that we have the most accurate and up to date information for your care. If you are unable to complete this registration process prior to your appointment, do not worry as we have in house tablets for you to use to complete the process. Please remember to bring the following items with you to each and every appointment:
- Government issued photo ID (driver’s license, state identification, passport)
- Insurance Cards
- Specialist Copay
- Insurance referral if needed
- Laboratory or Imaging testing related to your condition
- Current medication list
